2008 Illinois State Horseshoe Tournament Entry Form

The cost for entering is as follows; Men’s entry fee is $37 per entry; $25 entry fee, $11 scorekeeping for a 12 player class and the $1 for Junior Fund. Men’s 30’ entry fee is $35 per entry; $25 entry fee, $9 score keeping for a 10 player class, and $1 Junior Fund. Women’s entry fee is $35 per entry; $25 entry fee, $9 score keeping for a 10 player class, and $1 Junior Fund. Junior Class no charge. Doubles Tournament entry fee is $8

Check the appropriate boxes and mail entry form and fee NO LATER THAN JULY 31^st 2008. All pitchers must have an NHPA membership card

Doubles Tournament is now a walking mixed (men and women) doubles format. The 16 person classes will be sorted in a descending order basic on ringer %. The #1 person will be paired with the 16^th person in that class, #2 person will be paired with the 15^th person in that class, etc.

ALL PLAYERS MUST WEAR SHOE’S AND A SHIRT WITH THEIR NAME ON THE BACK. THE LETTERS MUST BE 1” IN HIEGHT AND MUST NOT BE IN MAGIC MARKER OR INK.
